Sliding-scale fees:  How this works

Everyone is welcome to get their medical equipment and health care supplies at our "GoodHealthwill."  Our sliding-scale fees for medical equipment and health care supplies are based on your total monthly household income (both earned and unearned income, from all sources before taxes) and your family size.  Fees pay for our rent, utilities, and insurance.  Find your monthly income and family size in the chart, then scroll down:

 If you are in category A, you pay 0% of the item’s fair market value.

Category A requires confirmation of income, such as by a case manager or social worker, or by showing eligibility for EBT, OAP, AND, AB, TANF, etc. 

If you are in category B, you pay 5% of the item’s fair market value.

If you are in category C, you pay 10% of the item’s fair market value.

If you are in category D, you pay 15% of the item’s fair market value.

If you are in category E, you pay 20% of the item’s fair market value.

If you are in category F, you pay 25% of the item’s fair market value.

Examples

You need a used walker with a fair market value of $40 (which would cost about $120 new).  Your income and family size put you in category D, so you will pay $6 for the walker.

Your grandma needs an adjustable hospital bed with a fair market value of $400 (which would cost about $900 new).  Her income puts her in category B, so either you or she will pay $20 for it.
